Please see the enclose information on the 990-N form that must be turned into the IRS within 5 ½ months after the end of your Chapter/State fiscal year.  For example, the fiscal year for Illinois ended May 31, 2008, so we must file the 990-N e-Postcard by November 15, 2008.

The following information will be required to file the e-Postcard:

·        Employer identification number (EIN), also know as a Taxpayer Identification Number (TIN).

·        Tax year

·        Legal name and mailing address

·        Any other names the organization uses

·        Name and address of a principal officer

·        Web site address if the organization has one

·        Confirmation that the organization’s annual gross receipts area normally $25,000 or less

·        If applicable, a statement that the organization has terminated or is terminating (going out of business)

To file electronic Form 990-N (e-Postcard): http://epostcard.form990.org/

Click Here to see the form for Quarterly Donation Summary Reports.  It is very important that we (chapters and states) get this information, so that we can have a total at the end of the year of monies/materials donated and hours.  We want a total of what we do for our communities and this wonderful organization.
